Welcome to the Evolution of Combat Barrows Guide!
Here are just a few recommendations that will make the Barrows a much easier task for you to complete. None of these are required, but all of them are strongly recommended in order to be successful and efficient.
Recommended Levels:
70+ in all three forms of attack (magic, ranged, att/str)
70+ Defense
Recommended Achievements:
Completed Myreque quest line (allows use of Drakan’s Amulet, quick travel from Canfis to Mor’ton, etc.)
Unlocked Burgh De Rott as a teleport option for games necklace (temple trekking)
Completed Morytania Hard tasks (grants double rune rewards and halves prayer drain)
Completed Ghosts Ahoy (ectophial)
Setup
There are several armor options, the best of which is void. The key to being successful with your armor setup is quickly switching methods of attack and/or defense. This puts void at the top of the list due to the need to switch only a few items (helmet and weapon). Try to use as much multi-combat equipment as possible! For example, even though an arcane stream necklace gives better magic bonuses than an amulet of fury, the fury is better due to its boost to all sides of the combat triangle.

The void setup is the most practical in terms of offensive output and saving inventory space, however it does not provide as strong defensive bonus. This is the setup that I use.
This next example consists of a full set of armor for each side of the combat triangle.

Note that godwars armor can be replaced by cheaper variants. This style setup provides protection from each brother individually and is a more safe method to use for beginners. This is, however, a much heavier inventory which might cut into your run, as well as a much more full inventory, not allowing for certain amounts of loot if too much food and potions are brought along.
This third setup is a hybrid setup of non-void armor which can be used by those who do not have void armor.

This is a much lighter setup, providing a mage based outfit with a piece of ranged and melee armour thrown in. Your attacks will not be as restricted as they were before the EOC whilst wearing armor of another attack style, making it easy to kill the brothers without much wardrobe change. Because 4 of the 6 brothers are weak to magic, that has been picked as the prime part of this outfit, however if you prefer a ranged or melee based setup, that is your choice.
A few last things to note about your setup:
Drakan's Medallion is used for teleportation directly to the barrows, as well as Burgh de Rott for a bank, and Darkmeyer for an alter. If you do not have a Drakan's Medallion, you may wish to replace it with a games necklace if you have unlocked Burgh de Rott as a destination from Temple Trekking, or an ectophial for teleports to Port Phasmatys.
You can use auras to improve your attack output, or to help extend your runs. Personally, I like to use Vampyrism, as it provides 5% health restore. Combining Vampyrism with Soul Split eliminates the need for food. Using Vampyrism alone might leave you needing a piece of food once in a while, and Soul Split is perfectly fine for healing by itself, but be sure to have plenty of prayer potions if you're using it. Penance is another aura that works, but not quite as well as it does in other places. If you work correctly, you might not take enough damage to make penance worth using.
Getting There
There are a few routes to choose from in order to get to the barrows. If you are starting from Burg de Rott, it is just a quick jog over to the barrows, as seen below.
If you are traveling from Canfis, the following two paths will get you there. The red path is the path you will have to travel if you have not completed "In Search of the Myreque" quest. If you have completed In Search of the Myreque, use the blue path, take the trap door behind the tavern in canfis, and follow through the swamp. This is a much quicker path.
Or, as described earlier, if you are using a Drakan's Medallion, you can teleport directly to the Barrows.
Fighting the Brothers
The Key to fighting the four melee using brothers (Dharok, Guthan, Torag, and Verac) is using the Impact magic ability. This ability acts as a bind, which will hold the brother in place for 20 seconds. Use this time to attack the brother from a distance.
(Use this to stun the brother)
If the brother is not killed by the end of 20 seconds, use the Asphyxiate ability to "force choke" him. Asphyxiate will stun the brother for the duration of the attack and should kill the brother. If he is not killed after using Asphyxiate, simply use the impact ability again.

Of course, Gold has smashing power of his own...
As you can see, Dharok has the capability to hit you HARD if you give him the chance. Using the Impact and Asphyxiate abilities prevent him from gaining that opportunity.
Karil and Ahrim will require a different method of attack, seeing as how Ahrim is more resistant to your magical attacks, and Karil will simply shoot you from across the room. Let's begin with Karil:
It is suggested to use a two handed melee weapon, such as a godsword whilst fighting Karil. The key to fighting Karil is defeating him as quickly as possible. Assault (Threshold ability) is very effective against Karil. It might be a wise decision to carry adrenaline over from a previous battle to fight Karil. Use the Freedom ability to keep yourself in combat while you run to Karil's barrow.
Ahrim is similar to Karil in that he has the ability to attack you from anywhere in the room, rendering the impact ability useless. Again, defeating him quickly is the best mode of discourse. You will want to use a ranged weapon, for example a crystal bow or Royal crossbow. Rapid fire (Threshold ability, similar to assault) is effective in defeating Ahrim quickly.
The following is a map showing which brother resides in which barrow.
*Note that Akrisae is accessible after completing the quest "The Ritual of the Mahjarrat." His tomb is connected to Verac's barrow, however, it is not suggested to fight Akrisae as he hits hard, accurately, prays against your attacks, and is generally a pain in the butt.
The Tunnel
While going from barrow to barrow, fighting the brothers, you will eventually reach a barrow which gives this message:
You will be given the option to enter the tunnel. It is suggested to kill all the other brothers before proceeding into the tunnel so as to avoid sudden high hits by brothers who may appear after opening a door. Once you enter the tunnel, you will appear in one of the rooms marked with a red X (remember which one it is, as you will have to return to this room on your way out if you don't have a teleport) on the diagram below:

Green marks represent doors that you MAY OR MAY NOT be able to open based on the layout of each specific tunnel (it changes after each looting of a chest.) Purple marks represent doors that might be locked with a puzzle question which you will have to answer to gain access to the chest room.
There are 4 different puzzles, shown below:
The circled answer is the correct answer. Note that the answer choices will appear in a different order, even if it is the same puzzle. If you get the answer choice wrong, your tunnel layout will change, and you will have to make your way through the tunnel to find the new puzzle door in order to access the chest.
Once you have made it through to the chest room, you will fight the last brother. You may encounter the brother when opening ANY door in the tunnel. If this occurs, simply defeat him on the spot and you will then not have to defeat him at the chest. Defeat the brother BEFORE looting the chest, otherwise the kill will not factor in to your chance of receiving loot!
Once you have looted the chest, make your way out via either teleportation or following your path backward, up the rope located in the room which you entered the tunnel through. You are now ready to start the process ALL OVER AGAIN!
A few notes
At frequent intervals while in either a barrow, or the tunnel, you will be haunted by a brother whom you have killed, giving an image similar to this:
When this happens, you will lose a chunk of prayer points, so keep a watch on them to make sure you have enough if you are praying.
Here is a list of the barrows brother's special abilities:
Ahrim the Blighted: Any attack may lower your strength level
Dharok the Wretched: His strength increases as his health decreases, being doubled when his health is 10%
Guthan the Infested: His attacks have a chance to heal him for 100% of the damage he inflicts
Karil the Tainted: Attacks have a chance to lower your magic level
Torag the Corrupted: Attacks have a chance to reduce your run energy
Verac the Defiled: More accurate hits through armor protection
Akrisae the Doomed: Attacks can steal your prayer
